Admissions

         A parent interview is required before admission into any of the above programs. The following forms must be signed, dated and on file: Emergency Contact, Parent Consent to Treat a Minor, Picture Permission, Childcare Monthly Contract, Emergency Transport, SmartCare Enrollment and proof of Immunizations. For infant enrollment, a Statement of Health and Feeding Schedule form will also need to be completed. It is the parent’s responsibility to advise the Center of any changes in address, phone number, e-mail, health history or immunizations.

No student will be allowed to attend until properly enrolled.

         No child will be denied admission on the basis of race, religion or handicap. We will do our best to accommodate children with
special needs. We retain the right to deny admission if all spots for a particular child’s age group are full or if we are unable to

appropriately accommodate a special need.
